refactor(cli): cover the rest of the `db` module with effect lint (CLI-2653) (#7049)

## TL;DR

brings the rest of the `db` module under the effect lint

## whats introduced?

- the per file `db-config.parse`, `db-config.toml-read`, `db-connection`
and `db-dns` entries collapse into `db-*.ts`, and `branch-target`,
`connect-errors`, `pooler-fallback` and `schema-flags` join, which
covers the whole `db` module
- `db-config.layer`, `db-pull-run` and `db-push-core` yield their errors
directly with the same classes and messages, and `branch-target`
recovers through `Effect.option` and `orElseSucceed`
- `db-config` integration tests feed env through `Config` instead of
`process.env` hooks, and `db-image` and `db-target-flags` move to
`it.effect`
