Address review of the reader-oriented docs (1.18.1)

- Merge master (1.18.0) and take 1.18.1, with a changelog entry.
- README: guide links are absolute, so they work on PyPI; links to the
  integration guide, legacy file registry, release notes and release
  procedure are back; old section anchors sit on the guides that replaced
  them.
- Home page and interface reference describe expire_after,
  return_stale_on_error, validator and fetch_bytes, no longer claim that
  only force=True refreshes, and no longer claim to list every entry point.
- The interface reference links to the site's own pages.
- docs.sh runs the documentation examples. The checker reads every page and
  pairs an example with output only when the output block directly follows
  it, so API signature blocks are not mistaken for output.
- Release notes in the site navigation.
