Py2JS: global declarations skip enclosing function locals (#503) (#504)

A name declared global in a nested function was only dropped from that
function's own locals, so emitName's outward scope walk bound it to an
enclosing function's local of the same name. Scopes now record their
global declarations and the walk stops there, resolving to module scope.

In program mode the module-level let for a global-declared name gets a
distinct JS identifier ($g$name), since JS cannot name a binding that an
intermediate local shadows.
