fix: use declared or host-supplied context item for global variable initializers (#170)

A declare context item now provides the focus for declare variable
initializers, so '.' there no longer reports XPDY0002. Hosts can pass
externalContextItem to runDiagnostics/checkContextItemUsage for the same
effect; the QT4 harness sets it when a test environment has a
<source role=".">. Function bodies are still checked.
