Document migrating from django-bootstrap3 (#892)

MIGRATE.md covered django-bootstrap4 only, while projects on Bootstrap 3
tend to go straight to 5. Adds a "From django-bootstrap3" section, based
on Meetnetten's migration of about 260 templates: renames, settings that
changed default or disappeared, the removed buttons, bootstrap_icon and
bootstrap_message_classes, renamed tag arguments, and the Bootstrap
class changes that take most of the work. The django-bootstrap4 section
is unchanged apart from moving down a heading level.

Closes #891.
