cleanup: drop unused const overloads and test-only readers

Nothing called the const twins of these accessors. The compiler uses
the non-const overloads. NodeRef's explicit operator bool was unused.

DomBits::none, IrStringTable::size, NameIntern::size, and
SymbolIdList's size, empty, and operator[] were only reached from
tests. Those tests now use the product readers.
