Validate DIGEST-MD5 qop before responding with qop="auth"

SaslMechanismDigestMd5 always responded with qop="auth" without checking
whether the server offered it. Per RFC 2831, the client must abort if it
recognizes none of the server's qop options (qop defaults to "auth" when
absent).

* Throw SaslException (InvalidChallenge) if the server's qop list does
  not include "auth".
* Parse the qop list case-insensitively and trim whitespace around
  comma-separated entries.
* Replace the FIXME in DigestResponse with a note.
* Add TestUnsupportedQop/TestSupportedQop unit tests.
* Document the fix in ReleaseNotes.md.
