fix(cypher): enforce ordinary and vector index duplicate admission

Reject equivalent definitions and globally conflicting index names with schema error classes. Preserve guarded metadata and vector runtime no-ops. Append the exact original reported statements to the shared differential corpus without displacing its protected prefix.
