feat: write VarDict's mean ALT mapping quality as FORMAT MEAN_MAPQ (#115)

MEAN_MAPQ is VarDict's MQ column: the arithmetic mean mapping quality of the ALT reads, uncapped, so a MAPQ of 255 counts as 255. It is not written as MQ because the VCF specification reserves that key for the RMS mapping quality of all reads at the site. It is FORMAT, Number=A, and missing when there is no ALT allele.
