feat: write VarDict's read-position evidence as MEAN_DIST_TO_READ_END and ALT_READ_POS_VARIES (#114)

MEAN_DIST_TO_READ_END is VarDict's PMean column: the mean 1-based distance from the variant to the nearer end of the aligned part of each ALT read, with soft clips excluded. ALT_READ_POS_VARIES is VarDict's PStd column, which is a 0/1 flag rather than a standard deviation: 1 when the ALT reads place the variant at two or more distances from the read end, and forced to 1 when VarDict merges realigned reads. Both are FORMAT, Number=A, missing when there is no ALT allele. VarDict's QStd flag is not written. The struct's doc comments for the two columns now say what they hold.
