fix: keep calls that differ only in END when dropping duplicate rows (#108)

VarDict writes a call twice when two BED regions overlap, and vartovcf keeps the first copy. The duplicate key was contig, start, REF and ALT, so two different SVs at one position with the same symbolic ALT, such as two DELs with different ends, collapsed into one. END is now part of the key. The duplicates fixture, which no test read, is now tested and carries such a pair.
