docs: describe how Fragment.Encode writes, and complete the CHANGELOG

Since #613, Fragment.Encode encodes all boxes up to the mdat payload into one pooled buffer and writes that with one call, then the payload directly, so a decoded fragment takes two Write calls instead of about one per box. The package documentation section "Writing files and segments efficiently" and the comments on File.Encode and MediaSegment.Encode still said that one Write call is made per box. They now describe fragments as the exception, and say that EncodeSW brings little for fragments, while a buffered writer still helps for the remaining small writes and for moov boxes. Fragment.Encode describes how it writes.

The CHANGELOG now says that the LevaLevel.GroupingType change is a breaking change, as the earlier breaking change in it does, and that Fragment.Prft is the first prft box of a fragment instead of the last since #612.
