opt: fold constant bitwise ops and shifts (#408)

Fold same-op And, Or, Xor, Shl, and Shr when both constants are foldable integers.

A zero xor, an and of all-ones, an or of zero, and a shift of zero become a copy of the variable and do not rewrite the constant. An and of zero and an or of all-ones become that constant. A shift whose count is negative, or whose distances sum to the width or more, stays two operations.
