Fixed ImapStream.Flush() to always flush the underlying stream when needed

ImapStream.Write() can drain its output buffer directly to the underlying
stream (when the buffer fills up or when writing block-sized chunks),
leaving outputIndex at 0. Flush() would then early-return without ever
calling Stream.Flush().

This was harmless while the underlying stream transmitted every write
immediately, but it is a latent deadlock for any underlying stream that
does buffering of its own. Track a 'dirty' flag so that Flush() knows
that bytes have been handed off to the underlying stream and that it
still needs to be flushed.
