subscriptions: cancel always ends the subscription (#911) (#916)

Subscription::cancel() ends the subscription on both clients, whether or
not a cancel is written to TWS.

- sync: shared releases the count; otherwise the cancel is written while
  TWS may still run the request; InternalSubscription::cancel always
  runs. Fixes order subscriptions, order_update_stream (slot freed),
  types without a cancel message, ended and snapshot-ended streams.
- async: shared `stopped` flag; every clone yields Err(Cancelled) at its
  next poll, then ends. Route still released at drop (#893).
- TickSubscription::cancel after the last batch writes nothing (both).
- migration-5.0 behavioral note; CHANGELOG.
