read/goff: add LogicalRecord (#1001)

Consolidate continuation record handling, and use it to implement
readobj support.

Implement 64-bit offset support for relocations, and make relocation and
length record parsing stricter.

Also read all records after the header as a single slice. We need to
read them all anyway.
