* Reconfigure a hybrid function whose file reaches tf only by a star import

A file whose decorator reaches TensorFlow through another module, as
with `from B import *` where `B` imports `tensorflow as tf`, has no
TensorFlow import of its own, so `getImportContext` found nothing. The
reconfiguration gate then reported the signature unwritable and the
function fell through to the already-optimal code 13, though a signature
was inferred.

The reconfiguration path now injects a `from tensorflow import ...` line
when the file has none, sharing the per-file record the fresh-decorator
path already keeps (#574), so a file that mixes both paths injects once.
The narrowing path (#808) uses the same context. The import-planning
pre-pass now also covers reconfigured functions, so the single injected
line carries every function's dtypes.

Where a signature was inferred but its names still aren't in scope, as
under `from tensorflow import function` alone, the function now reports
the new `INFERRED_INPUT_SIGNATURE_NAMES_NOT_IMPORTED` (code 33) instead
of code 13.

Fixes #1018.

* Say that a declined reconfiguration can still inject the import

The Javadoc of `reconfigure()` said a declined emission produces no
edit, but the injected import is added before the gate runs. It now
says that, in a file without its own TensorFlow import, the edits can
inject the import with no decorator change. Through the processor that
arises only for a narrowed literal whose span can't be resolved. Also
rewrap a comment the formatter split.
