fix(progress): fill the bar from step progress, not from finished items

The bar counted whole items, so with one item it never moved at all - which is
`rman changelog` under `changelog.groupBy: 'group'`: one file to write, the bar
empty at `0/1` for the entire run while the row beside it counted to 1825.

A finished item still counts as a whole one; a running item now counts as the
fraction of its own steps it has got through. The numbers were already there -
this just uses them. `run` gains the same thing in the small: a nine-step build
advances within the package instead of jumping at the end of it.

`renderProgressBar` clamps both ends, since a fractional input can round past
the width and `'x'.repeat(-1)` throws inside a redraw, where nothing catches it.

The `done/total` counter stays items. It is a different true fact - how many
files are finished - and the one the bar cannot carry.
