Redesign the web dashboard (#70)

* docs: add dashboard redesign plan and mockup

* feat(web): store job name and last run result

* feat(web): describe cron schedules in plain text

* feat(web): redesign dashboard as responsive job table with inspector

Replace the card/list views and the three modal dialogs with one job table,
a job inspector (runs, output, enable/disable, run now) and a native run
dialog. Filter tabs gain Disabled, search matches names, rows show the last
exit code and duration. Layout adapts via container queries: docked
inspector on wide screens, overlay drawer on tablets, full screen on
phones, 44px touch targets below 1180px.

htmx drives all updates; app.js is replaced by a small ui.js for focus,
dialog open/close and poll-failure marking. Expired sessions answer htmx
requests with HX-Redirect to the login page. Removed view-mode,
sort-toggle, filter-toggle and the old modal routes; the /api/v1 JSON API
and the curl run/toggle contracts are unchanged.

* fix(web): single empty-run message and theme toggle on phones

Verification of the redesign found two defects: the inspector for a job
without runs said so twice (runs list and output pane), and the theme
toggle was hidden below 600px with no other control to switch themes.

* fix(web): keep the plain hostname label compact on phones

Only the clickable server selector is a touch target; the 44px minimum
height made a plain hostname label look like a button.

* docs: describe the redesigned dashboard and refresh screenshots

* fix(web): keep dashboard state consistent across resizes, tabs and removed jobs

- recompute the inspector's inert covering on resize, so widening frees
  the table and narrowing keeps focus off covered controls
- return focus to the inspector heading when the run dialog closes and
  the job's row is covered or not shown
- send each tab's filter and sort with every list request; the cookies
  only set a fresh page's modes, so another tab's change no long... (continued)
