fix(openai): support gpt-6 models and allow disabling reasoning (#467)

gpt-6 models reject max_tokens, so treat them like gpt-5 and send
max_completion_tokens instead.

reasoning_effort "none" is the default and is persisted by save-config, and
models without reasoning support (gpt-4o-mini) reject the parameter, so
"none" keeps meaning "not sent". a new opt-in value "off" sends the API's
"none" to disable thinking on models that accept it (gpt-6-luna).
