docs(keymaster): describe polls as the vaults the code creates

Spec §10.3 described a poll asset with voters, ballots and vault fields
that Keymaster never writes. Replace it with what both ports do: a poll
is a vault holding its config, one ballot DID per voter under a salted
ballot key, and optionally published results. Document ballot filing,
publishing and reveal, the results shape and all 16 poll routes, and
fix the stale KeymasterInterface link.
