Pull #3017

github

Answer a common Accept header without copying the header versioner

Middleware::Base#call copied the header versioner for every request so
that #before could keep the env and the Accept header it scrubs in
ivars. The versioner already answers the Accept headers clients
usually send from a table built when it is (a declared media type, */*
or none). When the API is not strict, a request whose header is in that
table now has its media type recorded and the app called from the one
instance the stack built. Any other request, and every request to a
strict API, whose checks read the header itself, still goes through
#before on a copy.

A subclass overriding #before is therefore not called for those
requests; UPGRADING says to override #call instead.
