Preserve missing-cell positions in see() output for short rows (#726)

I found this while tracing the inspection code for the configuration
documentation. The short-row input is the same kind already exercised by
test_look_irregular_rows; this is not a claimed production incident.

There is also a cross-column case: replacing the header with (foo,0)
currently produces foo: a, , b and 0: 2. The missing entry goes into the wrong column.

For historical context, the positional-key change in a4b6fd8
fixed duplicate headers (#178 / #269) but left this exception branch keyed by name.
