Stop copying the auth middleware per request

Middleware::Base#call copied Grape::Middleware::Auth::Base for every
request so that #call! could keep the env in an ivar. Only the
strategy's block read it, through #context, to find the endpoint that
checks the credentials. The block now takes the endpoint from the env
it is built for, and #call answers from the one instance the stack
built. #call! still answers a request when called directly and keeps
its env, as Base#call! does.

A subclass overriding #call! is therefore no longer called for a
request; UPGRADING says to override #call instead. Strategies
registered with Auth::Strategies.add are middlewares of their own and
are not affected.
