Index fragments by name in the validator walker

The walker found each fragment spread's definition by scanning
Document.Fragments. It also walks every fragment definition on its own,
entering each fragment reachable from it. On a document whose fragments
spread one another, that made validation cubic in the fragment count:
1 500 fragments, each spreading the next two, took 5.4 s. With an index
built once per walk, it takes 0.3 s. Duplicate names still resolve to
the first definition, as ForName did.
