fix(mp4): bound streamed sample reads by the mdat payload

GetSample, GetSampleRange and GetSamples of the streaming SampleAccessor passed each trun sample size to MdatBox.ReadData, which allocates a buffer of that size before reading, so a 170-byte stream claiming 1 GiB samples made them allocate 1 GiB before failing with unexpected EOF. Each sample's range is now checked against the mdat payload first, as AppendSamples already checks the total.
