Transpile ``continue`` in for statements (#777)

We transpile the ``continue`` statement in the bodies of for-loops,
including in the branches of the switches nested in the loops.

The parse tree gets a ``Continue`` statement. It can not complete
normally, just like ``Return``, so that we refuse the statements after
it, and C#, C++ and TypeScript do not end a switch clause with an
unreachable ``break;`` after ``continue;``. The type inference refuses
a ``continue`` outside of a for-loop, since ``ast.parse`` accepts it.

The common meta-model ``for_statements`` covers a ``continue`` followed
by further statements in the loop body, and a ``continue`` in all the
branches of a switch in a nested for-range. It is live-tested in all
the targets.
