Fix dialyzer warnings instead of ignoring them by line number (#516)

.dialyzer_ignore listed warnings by file and line, so any change above an
ignored line broke the ignore and every PR touching those files had to
update it. The warnings are now fixed or suppressed where they happen:

* Mint.HTTP.put_proxy_headers/2 gets the @spec its siblings have.
* Mint.UnsafeProxy.put_proxy_headers/2 always raises, so its spec returns
  no_return().
* ExUnit is added to the PLT, since test/support is analysed in the test
  environment and calls it.
* The HTTP/1 test helper dispatches through Mint.HTTP.stream/2 instead of
  the opaque struct's module.
* The HTTP/2 test helper's assertions for a closed socket or a socket error
  expected three-element tuples that stream/2 never returns.
* Mint.HTTP1.add_body/3 keeps building its buffer as an improper iodata
  list and suppresses the warning with @dialyzer.
