[#478] Add design doc for the global dashboard question filter

- Design doc VIZ-027: a dashboard filter-bar question whose excluded
options remove registration datapoints from every widget, including
widgets on other forms in the same registration family
- Each widget's query excludes the registration datapoints with a
Postgres subquery; no id lists are passed between widgets
- Decision log D-1 to D-9: whole-datapoint exclusion on every widget,
latest-answer rule within the date range, NOT IN semantics that keep
unanswered datapoints, registration-question filters (checked: a
monitoring submission never writes back to registration answers),
  and repeatable groups
- Lists every query that doesn't go through get_base_monitoring_qs, and
a test fixture based on the flow sketch
- Adds the flow sketch under doc/design/images/
