chore: prepare the v5.0.0 release (#1594)

* chore(cmake): raise the fallback version to 5.0.0

The fallback only applies when the version cannot be read from a git tag,
which is the tarball and git-less build case. It sat at 4.8.0 through the
v4.9.x and v4.10.x cycles, so a source archive of the v5.0.0 tree would
have configured, installed and reported itself as 4.8.0.

* docs: record the v5.0.0 release in the CHANGELOG

The [Unreleased] section had never been stamped. It held 94 bullets, but only
12 of them belong to this release: the rest cite PRs from #559 to #1384, which
shipped in v4.6.0 through v4.10.1. Stamping the block wholesale would have
attributed five releases of work to v5.0.0.

So [5.0.0] carries the 12 in-range entries plus the parts of the release the
CHANGELOG never recorded at all -- the removals, the behaviour changes most
likely to move a caller's result, ereal on its limb type, and the late cfloat
and posit work. The remainder moves under an honestly labelled heading rather
than staying under "Unreleased", where a reader would take released work for
upcoming work.

Two corrections while stamping: the header-layering headline said "22 of 38
number systems", written mid-epic. Measured at release: 37 of the 44 number
systems ship a core.hpp, and 36 of those reach none of <iostream>, <sstream>,
<iomanip>, <ostream> or <istream> transitively. elreal is the exception, via
<memory>.
