Consolidate manual-chapter references into docs/README.md

Each docs/functions/*.md page had its own sentence citing which
chapter(s) of the original Expression Calculator manual it was sourced
from, duplicating/fragmenting that info across files. Move the
chapter mapping into a single table in docs/README.md (which already
links to the manual via HISTORY.md), and drop the now-redundant
sentences from advanced.md, general.md, logic.md, number-theory.md,
and numeric-integration.md — keeping any non-sourcing content (AGENTS.md
porting-convention links, MCalc.pas function references) in place.
