Measure workflow timeout as accumulated alive time (#726)

The runner gives a workflow a fresh full timeout whenever it resumes. A
run that repeatedly suspends and resumes can therefore execute longer
than its timeout, while counting the suspended interval would charge
time when no work is running.

The runner now stamps ticks when it processes them and journals those
stamps. The reducer accumulates elapsed time while the workflow is
active, and a `session_start` tick resets the accrual anchor at each run
or resume. On resume, the runner schedules the remaining budget and
times out immediately if that budget is exhausted before restored work
starts.

The runner drains due scheduled ticks on every loop iteration, so a busy
stream of completed steps cannot starve a timeout. At timeout delivery,
the reducer checks the accumulated budget and rearms the timer if it
fired early.

Old journals have no timestamps or session markers and replay with an
empty budget. Resume downtime and the interval between the last
processed tick and a crash are not charged to the budget.
