Restructure control loop around a recursive broker tree (#725)

The control loop keeps one flat broker per run. Worker maps, reducer
commands, and stream accounting address steps by bare name, so the
current structure cannot represent a broker nested inside another
broker.

This change makes broker state a tree and addresses steps by
`StepId(namespace, name)`. The root namespace is empty, and the runner,
reducer, state restoration, and stream accounting all traverse through
the namespace path. A run still has one root broker, with no way to
create a child yet.

Root step IDs still serialize as bare names. The new namespace fields on
ticks and commands are omitted from the journal at the root, and root
task names retain their previous form. Older JSON snapshots and journals
still load. The golden serialization suite also loads a DBOS workflow
argument pickle written by the previous version, then checks collection
routing and recovery of pending work and a suspended waiter.
