Drop the unused simplecov-lcov setup (#2998)

`.simplecov` switched to the LCOV formatter `if ENV['GITHUB_USER']`,
which GitHub Actions never sets, so since #2467 CI has never written
`coverage/lcov.info`. The Coveralls action uploads SimpleCov's own
`coverage/.resultset.json` instead, and Coveralls reads it in full --
line and branch coverage, with each file mapped to its repo path.

So the LCOV block never ran and the `simplecov-lcov` gem was only ever
loaded by it. Both go; SimpleCov keeps its branch coverage and spec
filter.
