Spec the error presenter's middleware and version branches

`ErrorFormatter::Base#present` had two branches no spec reached:

- A route's failure presenter is only looked up when the endpoint has a
  request, i.e. for an error the endpoint raised itself. An error
  thrown by middleware before the endpoint ran keeps the shape that
  middleware gave it. A spec now pins that, and the comment on the
  check says so: it read as if the lookup could not work there, which
  it can -- the error middleware sets the status first.
- The presenter is handed `version:` on a versioned request.
