Merge a middleware's headers with a guard clause instead of a case

`Middleware::Base#merge_headers` dispatched on the response with a
`case` whose implicit else -- a response that is neither a
Rack::Response nor an Array -- left the headers unmerged, and no spec
reached it: `call!` hands it what `after` or the app returned, which
the Rack SPEC makes a triplet unless it is a Rack::Response.

The Rack::Response branch is now a guard clause and the triplet the main
flow, so that unreachable third branch is gone.
