docs: add AGENTS.md and Claude Code worktree config (#354)

AGENTS.md is the canonical agent guide. CLAUDE.md is a symlink to it so
both names load the same file.
Ignore the root .claude worktree and local settings paths.
Add .worktreeinclude so new worktrees get the local .env.
