Spec a request body that is announced but not carried

`Grape::Middleware::Formatter#read_body_input` returns early when the
request has no `rack.input` (optional since Rack 3.1), and
`#read_rack_input` returns early when the input reads as empty. The
existing "body is nil" and "body is empty" specs send CONTENT_LENGTH 0,
so `body_given?` stopped them before either guard was reached.

The new specs announce a body (a positive CONTENT_LENGTH, or chunked
transfer encoding) that is missing or empty, and assert the request
passes through unparsed.
