Spec the error failsafe on an env without rack.errors

When an error response cannot be rendered, `Grape::Middleware::Error`
writes the failure to `rack.errors` with `&.`, so a server that leaves
the key out still gets the failsafe response. No spec reached that
branch: Rack::Lint rejects such an env and rack-test flushes
`rack.errors` after every request. The new spec turns lint off and calls
the API directly with an env built without the key.
