RELEASE_PLAN: only open work, and say where the plan lives

The file had drifted into a changelog: completed items with dates, a "Code
health" section whose body was a paragraph about what had been finished, and
a released-versions list doing the same job as CHANGELOG.md. That is the
record's job, not a plan's, and it made the ~20 open items hard to find.

Rewritten to contain only what is left. Release blockers pulled to the top in
order, the test-suite reduction written up as a section with per-file counts
and a stated target, and the two open questions that only existed in session
notes (balanced_accuracy in INCREMENTABLE_METRICS, whether one run should emit
all four 2x2 cells) added to the numbered decisions. Items that were agreed as
"log it, do not build it" now have their own section rather than sitting among
the todos looking like todos.

Convention going forward: when an item lands, delete it. Do not mark it [x].

The paper's remaining work is in cccpm_paper/PAPER_PLAN.md, which references
this file for the one task both own (external reference validation against
Shen MATLAB / GenCPM).
