Make C++17 the new baseline, and require CMake 3.16 (#580)

Fix #577 

- Raise the minimum CMake version from 3.5 to 3.16 to support modern C++
and the latest releases of GoogleTest.
- Require C++17 and test C++17/C++20 consumer compatibility.
- Start SQLiteCpp 4.0.0 development (version update) and update related
documentation.
