Commit Message

Checkout never got past `actions/checkout@v4`. The failure was:

fatal: Cannot fetch both d1cbf07… and refs/tags/v0.12.0 to refs/tags/v0.12.0
```

`v0.12.0` is an **annotated** tag. With `fetch-tags: true` and shallow depth, checkout tries to map the commit SHA into `refs/tags/v0.12.0` and also fetch the tag object into the same ref — git refuses that.

**Fix** in `.github/workflows/hex-publish.yml`:
- Drop `fetch-tags: true`
- On tag push, assert with `github.ref_name`
- On `workflow_dispatch`, `git fetch --tags` then `git describe`

Re-running [the failed job](https://github.com/thanos/rheo/actions/runs/35535412923/job/106144482256) will still use the **old** workflow on the tag. You need the fix on the ref you publish from:

1. Commit and push this workflow change to `main`
2. Either move the tag onto that commit (`git tag -f v0.12.0 && git push -f origin v0.12.0`), or run **Hex Publish** via `workflow_dispatch` with confirm `publish` from a commit that has the fix and is exactly tagged `v0.12.0`
