Run every endpoint's Rack stack through one shared lambda

The innermost app of an endpoint's Rack stack looks the endpoint up in
the env and runs it, so it closes over nothing -- yet build_stack
allocated a fresh lambda for it on every endpoint.

Hoist it to a single constant. That lambda answers to the class rather
than to an endpoint, so #run can no longer be reached as a peer -- and
it was only ever protected to be reachable that way. Make it public: it
is where the stack ends, it belongs next to #call and #call!, and it now
says in a doc comment what it expects of its caller. #execute and the
rest of the endpoint's internals stay protected.
