fix: close telemetry tracing gaps

Propagate request spans into blocking samples, link detached statement refreshes without retaining HTTP spans, and record response status.

Warn when OTLP configuration is ignored by a default build. Add mutation-verified regressions, all-feature docs.rs metadata, locked local validation, and 0.22.0 documentation updates.
