fix #39: preserve scrape tracing and make telemetry opt-in

Keep request context across scrape, per-database, and blocking tasks; link detached statement refreshes without retaining HTTP request spans and record response status correctly.

Make OTLP telemetry default-off for standard builds and releases, retain published compatible dependencies, and warn when endpoint configuration is ignored. Refresh dependencies and document developer opt-in, footprint measurements, and hung-sample export limitations.

Add mutation-verified regression coverage, validate both feature configurations, use locked local validation, enable all-feature docs.rs builds, and update the 0.22.0 changelog.
