refactor: roll the type declarations into a single dist/index.d.ts (#969)

Emitting the declarations one-to-one publishes the source layout. core,
gestures, handlers, state, utils and the rest are implementation detail, and a
consumer has no business reaching them or seeing them change.

tsc now writes to build/types and rollup-plugin-dts rolls those into a single
dist/index.d.ts, which is what "types" pointed at before the previous commit
had to redirect it to dist/src/. Declaration maps are turned off, since they
mean nothing once the declarations are bundled.

The internal types are inlined rather than dropped, so the public API is
unchanged: the default export, the five named exports and the
`declare module 'chart.js'` augmentation all survive. Nothing else is
exported.

Verified against the packed tarball from a throwaway project: it contains one
declaration file, the augmentation still type checks
(chart.zoom, chart.zoomScale, plugins.zoom and friends), and neither a deep
import nor an internal type is reachable any more -- the former fails with
ERR_PACKAGE_PATH_NOT_EXPORTED, the latter does not compile.
