chore(release): prepare v0.15.0

Namespace tuples and pull-based discovery are the substance of this release.
Cut the [0.15.0] CHANGELOG entry and bump the version constant.

Both are wire changes that fail quietly rather than loudly, which is worth
saying in the release notes: a peer asking for the old single-field
cmsf/clear matches nothing, and a subscriber that only listens for
announcements simply never hears one. v0.14.x stays the line for the
slash-in-one-field namespaces.

Adds [moqt-d18] alongside the existing [moqt-d11] and [moqt-d14], in the same
revision-pinned style, since the entry cites Section 2.4.1.
