project: ban the noun ask and the adjective leading

The verb table banned every inflection of `ask`, and the exemption for a
noun of the same spelling covered `the ask` and `a big ask`. Both are
corporate filler rather than a genuine noun sense. The rules now ban the
noun outright, with `request` as the replacement, and the prose-lint
detection preamble records that every hit on the word needs review
rather than an excuse. The existing verb grep already matches the noun.
No new pattern is required.

`leading` is banned as an adjective of rank or prominence, such as `the
leading provider` or `an industry-leading tool`. The rewrite gives the
concrete fact or deletes the word. The positional sense stays valid for
`a leading zero`, `leading whitespace`, and `a leading underscore`. The
ornamental word grep takes `leading` alongside the other filler, and the
preamble records the split.

`best-of-breed` joins the banned phrases, and the phrase grep matches
both the spaced and the hyphenated form.
