DOC: describe the checksum names and closing any store

The store model page said md5/ holds plain text files with a checksum
and left it there, which no longer says enough: the file is named for
the kind of record as well as the record, a store written before that
keeps both kinds under one name and is read by fallback, and there is an
explicit call to rename the ones that can be attributed. The how-to said
directory stores have no close(), which stopped being true when the
method moved to the base class.

Written by hand rather than regenerated. Running cogdocs rewrites every
cog block on the machine it runs on, and the checked in output was
produced elsewhere, so a full pass churns unrelated files with local
paths and interpreter versions.
