docs: describe Pulse and comms-gatekeeper as the only sources

Update .env.default, README.md, docs/ai-agent-context.md and
docs/openapi.yaml to drop every PRESENCE_SOURCE / ARCHIPELAGO_STATS_URL
/ core-status mention, describe Pulse (/about, /health) and
comms-gatekeeper (/hot-scenes) as the only sources, and document the
deploy order (after comms-gatekeeper serves /hot-scenes and Pulse's
HTTP surface is up) and rollback (previous image, needs
archipelago-stats alive).
